Hi Robyn, Ian, and Laura,

My thought is that FORM 1 would be placed on this page under the "Request a New MSI PI Group" tab (you can replace the text there with the form). Note that I have a short text blurb to list at the top of this form.

I also had a look at the form earlier and it presents nicely. But didn't submit, cuz I wouldn't have seen the sub. 

Regarding layout: 
  • – could you put the Title, First Name and Last name fields all on the same line, and easily? Not sure if that's a mission, but maybe you could show me how to do it, if it doesn't require styling/code to implement. Otherwise, a phase 2 effort :)
  • – Some fields have an outline and others don't, this is also happening on the helpdesk forms, which I hadn't noticed before... so probably not a big deal.. but I just thought to mention that as well:
